You MUST complete 2 steps:

1. BOOK your appointment time. Select an appointment time, fill out your information, and then click place order.

2. Log in or create an account on the Wisconsin Department Health Services COVID CONNECT 2 Website.

After you book your appointment with us, you will be directed to COVID CONNECT 2 where you MUST CREATE AN ACCOUNT or LOG IN to your account.

When you arrive at our pharmacy for your COVID test, please park on James Street or Ludington Street and call us at (920)623-2701 from outside. A staff member will come out to you with the COVID test.
